Immediate loading post-extraction implants on upper central incisors

The patient came to the dental office complaining of mobility of the upper central incisors. A very extensive endo-parodontal lesion was diagnosed, which was confirmed by RX got during the specialist examination. In agreement with the patient, a post-extraction positioning of the implants in guided surgery with an immediate prosthetic load was opted.

The patient was asked to get a CBCT necessary to plan the implant-prosthetic rehabilitation surgery and required for the production of the surgical template. 

As previously agreed, the atraumatic extraction of elements 11 and 21 was performed, trying to preserve as much as possible the residual alveolar bone, and a careful curettage of the alveolus was carried out. 

Afterwards, 2 implants with flap-less technique were placed. Both fixtures were inserted with a torque of 35 NCm, using a surgical contra-angle and then brought to a torque of about 50 NCm with a manual dynamometric key.

In the same surgical phase, the provisional prosthetic restoration was placed.

After the patient has been educated and motivated to get an accurate oral hygiene, he came every 6 months for medical supervision and hygiene sessions. 24 months after the prosthetic loading, he was in successful and unchanged aesthetic and parodontal conditions, compared to previous medical checks.
