Heartland Dental recently announced Conuity Care Coordination, an enhanced platform and suite of services designed to better connect general dentists, specialty providers and patients throughout the specialty dental care journey.
Today, only 15% to 30% of patients advised to seek follow-up specialty dental treatment adhere to that recommendation. This breakdown in the patient journey can impact patients’ overall oral health, oftentimes leading to more complex, invasive, painful, or expensive care later. Conuity Care Coordination™ combines technology and care coordination services to help manage specialty dental care from referral, scheduling, treatment and follow-up, strengthening collaboration between providers and helping patients stay connected to the treatment they need.
